Order Radiology Exams

Thank you for choosing Virginia Mason for your diagnostic imaging needs. Our goal is to provide convenience, quality and extraordinary service to both you and your patients. We are looking forward to working with your team. To schedule a patient for a diagnostic imaging exam, please follow these steps:

1. Print the diagnostic imaging request form.
2. Complete the request form.
3. Fax a cover sheet with your letterhead and the diagnostic imaging request form to the location where you would like to schedule the exam.
4. Call the Virginia Mason Radiology Department to schedule a date and time for the exam (see diagnostic imaging request form for phone numbers).
5. Print the appropriate patient instructions, write the appointment date and time at the top of the printout, and give to the patient to ensure they arrive prepared for their exam.
