Fighting injury with communication

The Center of Occupational Health Education (COHE) Alliance of Western Washington is one of six COHE’s across the state. Labor & Industries launched this initiative to bring together providers, employers, workers and administrators with the goal of promoting adoption of proven practices to get injured workers back on the job both safely and quickly. Health Service Coordinators are on-hand to help manage workplace injury treatment and recovery protocols, and facilitate ongoing communication between all involved.

Meet the Health Services Coordinators

  • Lisa Parker, MSW, CRC, CDMS, PGAP

    Health Services Coordinator

    Lisa comes to the COHE Alliance of Western Washington with over 30 years of experience in the field of Workers’ Compensation; this experience includes over 12 years working in chronic pain management. Lisa has been a registered Vocational Rehabilitation Counselor (VRC) with the State of Washington since 1989, and worked as a private sector VRC from 1989 to 2012. She was the owner of a private vocational rehabilitation counseling firm, where she worked for 30 years. Before coming to the COHE, she worked for an industrial rehabilitation and chronic pain management clinic as a Vocational Counselor, Community Reintegration Specialist, Provider Relations Specialist, and most recently as a Clinic Manager. Lisa has been actively involved with the rehabilitation industry association, WA Chapter – International Association of Rehabilitation Professionals (WA-IARP), since 2008, having served as Secretary-Treasurer, Chapter President, and Board Member at Large. In her position as a Health Services Coordinator, she will be assisting our US HealthWorks COHE partners to deliver the best practices for patients by providing optimal care and returning them back to work as quickly as possible. In her spare time, Lisa enjoys crafting, reading, and spending time with family and friends.

  • Candy Mansfield, MS, CDMS

    Health Services Coordinator

    Candy comes to the COHE Alliance of Western Washington with over 20 years of experience in the field of Human Services. Her experience includes 10 years in the field of workers compensation in Washington, as a Vocational Rehabilitation Counselor and a Case Manager at an Occupational Health Clinic, as well as additional experience as a social service provider with a variety of populations. She will be assisting our COHE partners to deliver the best practices for patients by providing optimal care and returning them back to work as quickly as possible. Any spare time you will find her reading, planning a quilting project, and spending time with family and friends.

  • Eric Swigart, BA

    Health Services Coordinator

    Eric comes to the COHE Alliance of Western Washington with over 20 years’ experience in worker’s compensation. From 1993-2003, he worked as a Vocational Rehabilitation Counselor (VRC), providing vocational early intervention, assessment, and vocational plan development/retraining services for injured workers in Washington state. For the past 10 years, Eric worked as a Medical Case Manager, assisting medical providers, employers, injured workers, and other L&I system stakeholders in resolving complex claim-related issues. In his current position as an HSC, he will assisting COHE providers with the Franciscan Medical Group and our partners, with the goal of providing the highest quality medical care to our injured patients. In his spare time, Eric enjoys reading, writing music, and spending time with his family.

  • Jennifer Farland, MSW, CRC, CDMS

    Health Services Coordinator
    Southwest Region

    Jennifer comes to the COHE Alliance of Western Washington with over 17 years of experience in worker’s compensation. From 1996-2013, she worked as a Vocational Rehabilitation Counselor (VRC), providing early intervention, assessment, vocational plan development/retraining services for injured workers in Washington state. She is also a licensed social worker in Washington and has a vast knowledge of connecting individuals, medical providers, and employers with resources available in Southwest Washington. As a Health Services Coordinator, she will be assisting our Southwest Washington COHE partners to deliver the best practices for patients by providing optimal care and returning them back to work as quickly as possible. In her spare time, Jennifer enjoys perusing antique shows and attending sporting events with her family.

  • Dawn Dunn, BA, CDMS

    Health Services Coordinator
    COHE Alliance of Western WA

    Dawn comes to the COHE Alliance of Western Washington with over 25 years of experience in Workers Compensation. She began as a Vocational Rehabilitation Counselor (VRC) providing vocational services and assessments on state fund claims for 13 years. She also has 11 years of experience working for a third-party administrator as a Return to Work Specialist assisting employers to identify appropriate modified duty options, writing job descriptions and job analyses, obtaining medical provider approval and assisting with the formal job offer process. She has experience working at the Department of Labor and Industries as a Vocational Services Specialist Two and as a COHE Case Manager/Health Service Coordinator and Workers Compensation Case Manager for an Occupational Health Department. There she gained further experience handling State, Self and Federal fund insurance cases. When Dawn is not working, her passion for time with family and friends leads her outdoors skiing at Crystal Mountain, camping, playing games with her grandchildren, dining, dancing or traveling with her significant other and friends.

  • Chris Condit, CDMS, PGAP

    Health Services Coordinator

    Chris comes to the COHE Alliance of Western Washington with over 15 years of experience in the field of Workers Compensation. His experience includes 13+ years in the field of workers compensation in Washington, as a Senior Vocational Rehabilitation Counselor, as well as additional experience as a Community Reintegration Specialist with a chronic pain program in Washington. He will be assisting our COHE partners to deliver the best practices for patients by providing optimal care and returning them back to work as quickly as possible. Chris spends his spare time playing soccer, hiking, camping, reading, and spending time with his family and friends.

  • Minett Fernandez, BS

    Health Services Coordinator
    COHE Alliance of Western WA

    Minett comes to the COHE Alliance of Western Washington with over 28 years in the healthcare field. As an experienced healthcare professional, Minett has over 25 years in an occupational medicine/urgent care setting supporting injured workers and employers with work-related injury claims and optimal medical care, in addition to over three years in a multispecialty clinic setting addressing patient care needs in ENT, orthopedics/sports medicine, and pain management. Minett’s contributions to healthcare also spans from a 33-year career in the Army and Air Force reserves at JBLM. He has also worked as a medical technician/EMT, certified medical assistant, and clinic manager during his career working in healthcare. As a member of COHE, Minett will promote proven best practices that will assist with facilitating and enhancing the ability of an injured worker to return to work in a safe and timely manner. Minett enjoys helping people and making a difference in their lives. He also enjoys sports, fast-walks and spending quality time with family and friends.

  • Ryan Manuel, BA, CDMS

    Ryan comes to the COHE Alliance of Western Washington with over 15 years of experience in Workers Compensation. He began as a Vocational Rehabilitation Counselor (VRC) providing vocational services and assessments for both state fund and self-insured employers. He also has six years of experience as a COHE Case Manager/Health Service Coordinator and Workers Compensation Case Manager for an Occupational Health Department. There he gained further experience handling State, Self and Federally funded insurance cases. When Ryan is not working, his passion for landscape photography leads him outdoors to hiking and camping adventures.

  • Jamie Hodge, MA, CDMS, PGAP

    Jamie Hodge has been a Vocational Consultant since October 2003 assisting employers, providers and workers in return to work efforts as part of the worker compensation system. Jamie is a Certified Disability Management Specialist (CDMS) as well as a PGAP® (Progressive Goal Attainment Program) certified provider. Additionally, she is familiar with LEAN and Six Sigma for continuous improvement initiatives, innovative problem solving and project management. Prior to vocational services, Jamie worked as an Admissions Director providing career guidance for working adults and collaboration with employers to improve their workforce skills and reduce human resource management risk. Jamie has a BS in Zoology with an emphasis in pre-veterinary medicine and animal behavior. She completed her Master of Arts in Organizational Management with a focus on industrial psychology focusing on communication tools and techniques used by sustainable organizations and individuals. Outside of work, Jamie loves time with her family and friends. She enjoys reading, time with animals, adventures outdoors and photography. She thrives when she is traveling locally and abroad whenever possible.

  • Jack Lintelmann, M.Ed, CRC, CEASII, CBIS

    Jack comes to the COHE Alliance of Western Washington with over 26 years in the field of Workers Compensation as a Senior Rehabilitation Counselor, Vocational Rehabilitation Counselor Trainer, and Firm Owner. Jack has specialized in complex rehabilitation cases including Complex Regional Pain Syndrome (CRPS), amputations, and Traumatic Brain Injuries (TBI). Jack is a Certified Rehabilitation Counselor, Certified Ergonomic Assessment Specialist, and Certified Brain Injury Specialist. Prior to COHE he worked in a Multi-disciplinary Pain Management Program as well as a Multi-disciplinary Brain Injury Rehabilitation Program.

  • Terrence Hodge, MBA, LSSWB

    Terrence comes to the COHE Alliance of Western Washington with over 15 years of experience in Program management and Workforce Development for the State of Washington, Pierce and Thurston County and the City of Tacoma. Terrence spent five years as a Program Manager working on contracts with the Dept. of Labor, Schultz foundation, Whisper Foundation (Milgard), The Call of Duty Endowment, Walmart, Boeing, and Wells Fargo to develop return to work programs and partnerships for workers with disabilities, Veterans, and their family members. Before coming to COHE, Terrence worked for the Tacoma Pierce County Chamber of Commerce developing partnerships with key government, community, and business leaders. Terrence has worked at the Pentagon and served in the United States Army for 10 years (as a Topographer and Forward Observer) serving in numerous deployments during his enlistment. Terrence is a member of the Virginia Mason Franciscan Health Toastmasters, an advisor for the Tacoma Big Brothers Big Sisters chapter, and a member of the Tuskegee Airman Sam Bruce Chapter. His spare time typically involves enjoying a sports activity, travel/sightseeing, and spending time with family.
