Transforming Postpartum Care: A Bold New Program to Save Lives

The U.S. has the highest maternal mortality rate among developed nations, with rates more than doubling over the past 20 years. Black women, in particular, face disproportionately higher risks compared to their white and Hispanic counterparts, according to the CDC. Addressing these alarming statistics, Dr. Leizl Sapico, an obstetrician-gynecologist at Virginia Mason Franciscan Health (VMFH), has pioneered a groundbreaking initiative to improve postpartum outcomes.

Dr. Sapico created the Postpartum Wellness and Recovery Program (PWR)—the first of its kind in the nation—through a grant from the Mission and Ministry Fund at CommonSpirit Health. This three-year initiative focuses on tackling the leading causes of postpartum complications: hypertensive disorders, perinatal mood and anxiety disorders (PMAD), and gestational diabetes.

Why Postpartum Care Matters
"The postpartum period, often called the ‘fourth trimester,’ is critical but frequently overlooked," Dr. Sapico explained. "Most maternal morbidity and mortality during this time are preventable." A Washington state review revealed that most pregnancy-related deaths occurred postpartum, with maternal suicide being the leading cause—a trend mirrored nationwide.

The PWR program aims to change that by providing tailored, compassionate support for new mothers and parents, ensuring they feel seen, heard, and empowered.

How the Program Works

  • Phase 1: Hypertensive Disorders
    Launched in 2023, Phase 1 focuses on managing hypertension, a leading cause of maternal complications. Patients diagnosed with hypertension during pregnancy are referred to the PWR program for comprehensive follow-up care.

    • Patients receive blood pressure cuffs and education on warning signs.

    • Follow-up includes a clinic visit within 2–3 days, virtual check-ins, and regular communication with perinatal nurses.

    • The program tracks patients’ cardiac health for six weeks before coordinating a warm handoff to primary care providers and cardiologists.

  • Phase 2: Perinatal Mood and Anxiety Disorders (PMAD)
    This phase connects patients with support groups, reviews medication regimens, and provides a safe space for mothers and parents experiencing postpartum mood disorders.

  • Phase 3: Gestational Diabetes
    Set to launch in July 2025, this phase will support individuals managing gestational diabetes, focusing on long-term health and future pregnancies.

By addressing these critical postpartum challenges, the PWR program not only improves immediate outcomes but also promotes lifelong health. "This program is transforming how we view postpartum care," said Dr. Sapico. "It’s about educating, empowering, and preventing complications."
