Please arrive 15 minutes before your scheduled appointment time to fill out paperwork. 

What to bring:

  • Valid photo ID

  • Insurance information

  • List of additional current medications and dosages (including prescription and over the counter)

  • List of additional herbal supplements and remedies

  • Copayment

Virtual visit 
If the above appointment type is Virtual Visit, check your email for instructions on how to join your virtual visit and any additional information regarding your upcoming visit.
Please join 10 minutes early to allow time for set up.

  • Be sure you will be in a place with a strong WiFi connection.

  • For your safety, limit distractions and do not drive during your visit. It is an audio and video call.

  • If the clinic staff determines that the patient needs to be seen in-person, they may contact you to reschedule for an in-clinic appointment.

  • To avoid a fee, we require a 24-hour notice to cancel or reschedule any appointment.

Language assistance:

  • If Spanish is your first language, you will have the opportunity to provide this information when you arrive for your appointment instead.
