Transitional Service

Franciscan Transitional Clinic is designed to assist patients with their transition from their hospital stay or Emergency Services department visit and the immediate need for related follow up care and or further testing.

Franciscan Transitional Clinic will not participate in the management of chronic pain.

Patients referred to the Franciscan Transitional Clinic are often seen for one to three visits, and up to six weeks following discharge from an inpatient stay or an Emergency Services department visit, in order to ensure access for necessary follow up care and assistance with establishing care with a primary care provider within the community.

Franciscan Transitional Clinic will address medical problems that contributed to the patient's hospitalization or Emergency Services department visit but is not designed to provide long term management of chronic medical conditions.
